CMOM – CIS Housing Program (Med-QUEST Hawaiʻi)

Compassion • Stability • Hope for Hawai‘i Families

The CIS Housing Program, operated by Chaplain Ministries of Maui (CMOM), provides housing case management, supportive services, and specialized assistance for Domestic Violence (DV) victims across the State of Hawai‘i.
We proudly serve members enrolled through Med-QUEST, partnering with insurance providers and community agencies to ensure safe, stable housing for those in need.

Our Mission

To empower individuals and families experiencing homelessness, crisis, or domestic violence by providing compassionate case management, housing navigation, and wraparound support that restores stability, dignity, and hope.

Program Overview

CIS Housing Program (Med-QUEST)

The CMOM CIS Housing Program offers no-cost housing case management services for Med-QUEST–eligible members, including:

✔ Housing Case Management

  • Housing applications and placement

  • Rental assistance navigation

  • Shelter and respite referrals

  • Ongoing case management support

  • Court support letters & documentation

  • Housing stability planning

✔ Crisis & Support Services

  • Assistance after displacement or homelessness

  • Safety planning for vulnerable individuals

  • Referrals to mental health, medical, and spiritual support

  • Community resource connection

✔ Support for DV (Domestic Violence) Victims

CMOM provides confidential, trauma-informed housing support for DV survivors:

  • Emergency safety planning

  • Immediate safe housing referrals

  • Case management and navigation

  • Coordinated Entry and shelter support

  • Connection to DV crisis centers, legal advocacy, and counseling

  • Supportive documentation (housing, court, benefits)

  • Long-term housing stability support

Your safety and privacy are always the top priority.

Who Qualifies

You may be eligible if you are:

  • A Med-QUEST (Hawaiʻi Medicaid) member

  • Experiencing homelessness or housing instability

  • A survivor of Domestic Violence

  • Exiting incarceration, foster care, or crisis situations

  • Displaced due to fires, disasters, or unsafe conditions

  • Needing support navigating housing systems in Hawaiʻi

How the Program Works

1. Intake & Assessment

A CMOM Housing Navigator conducts a full intake and identifies housing needs, safety issues, and available resources.

2. Housing Plan & Navigation

We work with the member to develop a personalized plan and begin the housing search or safety process.

3. Application & Placement Support

We assist with all required paperwork, landlord communication, shelter placement, and referrals.

4. Ongoing Case Management

Members receive monthly support, outreach, documentation, and follow-up to ensure long-term stability.

5. Transition to Independent Stability

Once housed, CMOM continues providing support to help members maintain success.

CMOM collaborates with:

  • Med-QUEST Hawaiʻi

  • AlohaCare, HMSA, UHC, Kaiser, and Ohana/CCS

  • DV shelters & safe houses statewide

  • Local agencies, churches, and community organizations

  • Housing programs, respite centers, and group homes
